Community Health Priorities and Implementation Strategy
Every three years, Beth Israel Deaconess Hospital–Plymouth (BID Plymouth) conducts a Community Health Needs Assessment (CHNA) to identify and prioritize the most significant health needs in the communities we serve. This assessment helps guide our community benefit programs, partnerships, and investments, and ensures alignment with federal requirements for nonprofit hospitals.
The CHNA is informed by a comprehensive review of quantitative data, direct community input, and collaboration with local organizations and public health partners. Together, these insights help BID Plymouth focus resources where they can have the greatest impact on health equity and community well-being.

Our health equity priority areas include: equitable access to care, social determinants of health, mental health and substance use, and complex and chronic conditions.
As part of the Community Health Needs Assessment, we focus on groups that may face greater barriers to good health:
- Youth
- Individuals with disabilities
- Older adults
- Racially, ethnically, and linguistically diverse populations
- Low resource populations

These reports share the most recent health needs identified through the Community Health Needs Assessment and outline the actions planned over a three-year period to address them.
