Your Patient Portal
If you're a patient at Beth Israel Deaconess Hospital–Plymouth, you can easily access your health and medical records 24/7 through MyBILH Chart. This is Beth Israel Lahey Health's (BILH) free and secure patient portal to support your care.
With a MyBILH Chart account you can:
- Message with your doctor’s office and care team
- Review test results
- View your provider’s notes and visit summaries
- Access your health information
- Manage your appointments
- Request prescription refills
- Pay your bill
- Complete check in before your appointment
Register for MyBILH Chart
The easiest way to create a MyBILH Chart account, is to use the MyChart activation code that is available to you in one of these ways:
- During an in-person office visit with a primary or specialty care provider
- Printed on your after-visit summary
- Printed on your billing statement
Once you receive an Activation Code, visit MyBILH Chart to register for your online patient portal account.
If you do not have an activation code, you can follow the instructions to sign up without one.
