Your First Appointment
Please bring these items to your first appointment:
- Your insurance and/or hospital card
- A list of current medications and dosages
- Diagnosis details, including any MRI and CT scans, pathology reports and doctors' notes
Your insurance company may require referral from your primary care physician. Please confirm and submit your referral before your first appointment.
You may also be required to make a copay at the time of service. Please confirm this with your insurance company ahead of time and be prepared to make that payment when you arrive at our office.
Each referring provider is considered an important member of our team and will be regularly notified of the patient's status.
